notification

employees who have been injured, please follow the steps outlined below:

1. follow procedures! follow the host employer’s appropriate procedures, which may include logging the incident in a register of injuries and cooperating with relevant direction, aid and investigations.

2. contact workcover@jobwire.com.au as soon as possible! advise your jobwire representative of your injury and whether or not you are able to attend work.

3. seek medical attention! particularly if you are unable to attend work, and obtain a "certificate of capacity" from your attending physician.

4. authorise medical release! authorise the release of medical information to WorkCover for purposes of injury management and return to work (worker’s declaration on the compensation claim form for workers and employers).

5. arrange an appointment! meet with your jobwire representative as soon as possible after the injury to discuss the facts of the injury and complete appropriate paperwork.

6. fill out the paperwork! if you wish to claim for compensation, you will need to complete the "worker’s" portion of the compensation claim form for workers and employers. claims should be submitted as soon as possible. claims submitted more than 28 days after you have first attended your doctor may cause delay in WorkCover making a decision.

7. provide information! provide all forms including the WorkCover medical certificate to your jobwire representative along with any other information relevant to the matter, including any medical bills. you will be required to complete an "employment declaration form" for tax purposes if you lost work time due to your injury.

8. actively participate in, cooperate and comply with a medically approved injury management plan and return to work program, including any assessments required. this also includes maintaining contact with jobwire when off the job. failure to comply can suspend compensation payments.
